Hall of Fame Nomination Form

The Employee Hall of Fame is a new tool to honor your coworkers for a job well done. If you know of someone in your department who has met most, if not all, of the following criteria, please nominate them for the Hall of Fame.


Suggested criteria for Employee Hall of Fame includes:

    1. Treats individuals supported with dignity, respect and consistently exhibits Person-Centered principles.
    2. Has good attendance to work, meetings and trainings. Attendance is punctual, employee exhibits enthusiasm and participation and training is completed in a timely manner (includes training on Essential Learning).
    3. Possesses a strong knowledge of job responsibilities and agency policies/procedures and performs job responsibilities as expected within agency policies/procedures.
    4. Demonstrates that he/she is willing to go the extra mile to ensure the job is completed.
    5. Consistently demonstrates with individuals supported, family members and staff a positive attitude, willingness to work and good communication skills.
    6. Demonstrates confidentiality.
    7. Provides documentation that is person-centered and is completed in a timely manner.
    8. No medication errors or written reprimands within the previous month.
